For over 30 years Kenneth has been assisting individuals and families to work towards improving their health and wellbeing outcomes especially in later years. The focus is on preserving and enhancing quality of life.
This is achieved by:
- Assisting you and your family to make significant life decisions.
- Providing mediation and conflict resolution around decision-making in the family when issues arise
- Providing information about your rights and entitlements, available resources, and complaints mechanisms
- Advocating with you or on your behalf
- Liaising with your medical and allied health specialist in order for you to access necessary supports and stay as healthy and safe as possible
- Working with My Aged Care and the services they provide to ensure you are accessing the supports available
- Complete assessments and make recommendations and referrals to service providers that you may be eligible to be supported by.
- Providing counselling to individuals, families and groups
- Providing practical guidance and advocacy, e.g. assistance with housing, appointments, moving into residential care
- Coordinating formal and informal supports
- Supporting carers in their role and accessing practical support for them
- Assisting you to access community services, e.g. housing and government-funded services
- Assisting you with reading and completing paperwork
- Writing reports and support letters to assist you in accessing required services
- Assisting you to navigate aged care systems
- Providing key information about government benefits
- Assisting you with completing, updating your advanced care directives.
- Assisting family and loved ones to navigate the guardianship tribunal system if necessary
- Spending time and assisting loved ones when a family member is considered to need palliative care supports.
- Providing coordination and case management support and working with overseas and interstate family and power of attorneys.
